Information Collected

New Jersey Bariatric Center®’s policy is to protect the confidentiality of the individually identifiable information that you provide, and we limit access to that information only to those with a need to know. 

Information You Provide Us: We receive and store information you enter on our site or give us in any other way such as name, email address, phone number, etc..  This includes information you submit on Contact Us forms, Request an Appointment forms, and Online Scheduling as well as when you subscribe to our newsletters and e-mail updates.

Browsing Information we Collect Automatically: When you interact with our sites, we automatically collect certain information about your use of our sites. This information includes computer and connection information, such as statistics on your page views, traffic to and from our sites, referral URL, ad data, your IP address, and device identifiers.

For the purpose of enabling better website design and a better user experience, we employ google analytics (a web service by Google) to track site traffic and provide aggregate reports on how the public interacts with our websites.

Secure Communication

The Contact Us and Request an Appointment forms and email functionality on our site does not provide a completely secure and confidential means of communication. It's possible that your communication may be accessed or viewed by another internet user while in transit to us. If you wish to keep your communication private, do not use our forms or email.  For private, HIPAA compliant communication please contact us at 908-481-1270 for access to our HIPAA compliant communications system.

Unsubscribing

Our email and newsletter communications all provide a way to unsubscribe at the bottom of each email communication or newsletter if you decide you no longer want to receive communication from us.

You can also unsubscribe by emailing info@njbcenter.com with the name of the communication you would like to stop as well as confirming the email address you would like to remove.

Use of Information Collected

We use the information collected for things like:

• Optimizing the performance and user experience of our sites
• Operating, evaluating and improving our business
• Fulfilling requests for products, services or information
• Conducting research and analysis
• Establishing and managing your patient portal account with us

Data Retention

We will retain your information for as long as your account is active or as needed to provide you services, comply with our legal obligations, resolve disputes and enforce our agreements.

Information We Share

New Jersey Bariatric Center®’s policy is to prohibit the unlawful disclosure of your personal information.  New Jersey Bariatric Center® will not release your name or otherwise publicize the fact that you purchased goods or other information given to us unless: (a) you grant New Jersey Bariatric Center® permission to do so; or (b) New Jersey Bariatric Center® is requested or required to do so by law.

We may share the browsing information we collect about you with third parties who we have engaged to help us provide our services to you or to maintain our digital presence, such as our web developers who we have engaged to help us provide the services. In each case, we will ensure that these third parties have agreed to safeguard your data. The data reflects site-usage patterns gathered during visits to our website each month, but they do not contain information that personally identifies you unless you have given us permission to share that information.

We do not share or disclose personal information to third parties for marketing or sales purposes.

We may use or disclose your personal and browser information if required to do so by law or on the good-faith belief that such action is necessary to (a) conform to applicable law or comply with legal process served on us or our sites; (b) protect and defend our rights or property, the sites or our users, or (c) act to protect the personal safety of us, users of the sites or the public.

Tracking

Your browser or device may include "Do Not Track" functionality. At this time, we do not respond to browser "Do Not Track" signals.

Since we link to media and social media sites, other parties may collect your personally identifiable information about your online activities over time and across different web sites when you visit the site.

Security

The New Jersey Bariatric Center® site uses encryption technology, such as Secure Sockets Layer (SSL), to protect your personal information during data transport.  However, New Jersey Bariatric Center® cannot ensure or warrant the security of any information you transmit to us, and you do so at your own risk. We have taken reasonable steps to ensure the integrity and confidentiality of personally identifiable information that you may provide. In order to verify your identity, we may require you to provide us with personal information prior to accessing any records containing information about you.

Children and Minors Privacy

New Jersey Bariatric Center® makes no active effort to collect personal information from children under the age of 18, we do not wish to receive information from children under the age of 18 and we do not knowingly collect personal information from children or minors.
