Gastric Sleeve and Weight Loss - 4 Facts

Estimated reading time: 3 minutes

The gastric sleeve, also known as the vertical sleeve gastrectomy (VSG), is one of the most popular weight loss procedures in New Jersey, and the most performed weight loss surgery here at New Jersey Bariatric Center.

At New Jersey Bariatric Center we get a lot of questions about gastric sleeve surgery, also known as the vertical sleeve gastrectomy (VSG), from how it helps you lose weight to the health benefits for the procedure. If you’re considering a gastric sleeve surgery, here’s 4 facts to know about gastric sleeve:

1. Gastric Sleeve Helps You Lose Weight in 2 Ways

First, gastric sleeve surgery jumpstarts weight loss by removing a portion of the stomach and narrowing the remaining part of the stomach into a thin “sleeve-like” tube. The reduced stomach size holds less food so you feel full faster, while still allowing for the digestion of key nutrients. The second way is that the gastric sleeve procedure reduces the secretion of ghrelins in your stomach. Ghrelins are hormones responsible for making you feel hungry. Patients on average can lose about 40-70% of their excess body weight after a gastric sleeve procedure.

2. The Health Benefits Go Beyond the Numbers on the Scale

Not only does the gastric sleeve help you to lose weight, but it can also offer other health benefits and improvements for obesity-related conditions. In a study by the American Society for Metabolic and Bariatric Surgery (ASMBS), after gastric sleeve surgery:

As with any surgery there are side effects and risks to weight loss procedures. To learn more, visit our Side Effects & Risks page.

3. Gastric Sleeve Surgery is performed Laparoscopically or Robotically-Assisted

During gastric sleeve surgery our surgeons make small incisions in the abdominal wall to remove 75-80% of the stomach. This reduces the capacity of the stomach to 3-4 ounces, resulting in patients feeling full, faster on a smaller portion of food. At New Jersey Bariatric Center the surgery is performed either laparoscopically or is robotically-assisted, and usually lasts approximately 1-2 hours.

4. Hospital Stays are Generally 1 Day

Both methods - laparoscopic or robotic-assisted - of surgery involve smaller incisions through the stomach, which often leads to, for many patients, less pain and a short, 1 day hospital stay, in general. Zepbound® vs. Wegovy®: What the New Study Results Mean for Your Weight Loss Goals

A recent study about two leading weight loss medications - Eli Lilly’s Zepound® and Novo Nordisk’s Wegovy® has many of our medical weight loss patients at New Jersey Bariatric Center asking us, which weight loss medication is best for me?  Here’s a quick look at how the head-to-head study of Zepbound® vs Wegovy® reveals significant differences in weight loss outcomes between Zepbound™ and Wegovy® that could impact your weight loss goals.

Key Weight Loss Findings

The head-to-head weight loss study*, involving more than 750 participants with obesity or weight-related complications, demonstrated key results:

This represents a 6.5 percentage point difference in weight loss effectiveness.

*Note: Eli Lilly, the maker of Zepbound®, funded the weight loss study.  Full study results can be found here.

Understanding How Zepbound® & Wegovy® work

Both Zepbound® and Wegovy® are injectable medications designed to address obesity by suppressing appetite, slowing stomach emptying, modifying hunger signaling pathways and supporting long-term weight management. While the study shows Zepbound®'s increased weight loss potential, individual results can vary. Factors influencing weight loss include individual metabolism, adherence to medication, lifestyle modifications and overall health conditions. These medications are not quick fixes but tools in a comprehensive weight management strategy.

Important Considerations for Patients

While these results are impactful, at New Jersey Bariatric center we emphasize that medication is just one component of a successful weight loss journey. With over 20 years of helping patients reach and maintain their weight loss goals, we know that medications like Zepbound® and Wegovy® are not a standalone solution. They are most effective when combined with consistent lifestyle modifications, balanced nutrition, regular physical activity and ongoing medical supervision. When trying to decide between Zepbound® versus Wegovy®, align your medication choice with your weight loss goals and consider your insurance coverage as coverage for each medication may vary.

Switching from Wegovy® to Zepbound®

If you're currently on Wegovy® or other weight loss medication and want to switch to Zepbound®consult with your healthcare provider. They'll assess your health, goals, and medication transition options. Patients experiencing a weight loss plateau with Wegovy® may be good candidates for switching to Zepbound®, pending medical approval. Your doctor can determine whether Zepbound® is the right choice for you, help you safely navigate the switch, ensuring minimal disruption to your weight loss journey.

Long-Term Weight Management

Weight loss medications are tools for managing obesity, not permanent solutions. Weight regain is possible when medications such as Zepbound® and Wegovy® are discontinued, especially if previous lifestyle habits resume. Weight loss is a journey, not a sprint. Developing consistent healthy eating habits and using NJBC’s services such as Tuesday Talks monthly support group meetings and virtual fitness classes can help you maximize your weight loss.

Making Your Decision

While the study suggests Zepbound® might offer more weight loss, individual experiences differ. The most important step is having an in-depth conversation with your healthcare provider to determine the best approach for your unique health journey.

Weight management is a personalized process. Both weight loss medications are powerful tools, but lasting success comes from a holistic approach to health and wellness.

How to Pick The Right Bariatric Surgeon in New Jersey Who's Right For You

Starting your bariatric (weight loss) surgery journey in New Jersey can be overwhelming.  Questioning “how to find the right bariatric surgeon for me?” is common.  As is how do I find the right bariatric surgeon near me?  Whether you’re considering bariatric surgery - gastric sleeve or gastric bypass - for the first time, or you’re interested in a revision procedure, this guide can help. We’ll dive into key factors to consider when choosing a bariatric surgeon. When you’re informed you can feel empowered to select the surgeon who best meets your needs and is in sync with your weight loss goals.

Finding The Right Bariatric Surgeon

There are many bariatric surgeons in New Jersey, and finding the right one for you is not only about searching Google for their credentials and experience, it’s about looking a little deeper and asking the right questions. What is the surgeon's approach to patient care? Do they provide comprehensive weight loss care, including nutrition and lifestyle counseling? Is pre-operative and post-operative bariatric surgery support a part of their care plan? The quality of the hospital can affect your comfort and safety. Knowing how to pick the right bariatric surgeon for you can ensure that you are making an informed decision.

Check Surgeon Credentials and Experience

Choose a bariatric surgeon who is board-certified in general surgery with specialized fellowship training in bariatric surgery. Ensure that they are accredited. Verify their credentials and experience through state medical boards or professional organizations like the American Society for Metabolic & Bariatric Surgery (ASMBS). Low complication rates indicate a skilled bariatric specialist. And look for a surgeon with a better-than-average complication and mortality rate.

Schedule A Consultation

A personal consultation allows you to assess your compatibility with the surgeon. Look for qualities like patience, empathy, and a willingness to clearly explain the surgical process. Ensure the surgeon takes the time to understand your medical history and weight loss goals. Look for a surgeon who gives you straightforward answers to the tough questions and discusses both the benefits and the risks when you are figuring out how to pick the right bariatric surgeon near you.

Ask About Surgeon's Approach

Some surgeons take a comprehensive, multidisciplinary approach addressing both physical, psychological and lifestyle aspects of obesity. Others focus solely on the physical. Choosing a surgeon whose philosophy aligns with your needs is essential for long-term success. A multidisciplinary approach involving nutrition and mental health counseling is often key to sustainable weight loss.

Commitment to Long-Term, Sustainable Weight Loss

Will the surgeon work with you for the long-term? Cheering on your successes, encouraging and collaborating with you during the tough times will go a long way. Weight loss is a journey, not a sprint. Look for a surgeon who will be there to coach and motivate you after surgery. One who has a weight loss team with nutritional and exercise support as you adapt to your new digestive system.

Hospital Affiliations and Facility Quality

The quality of the hospital where the surgery will be performed is an important factor. High-quality, accredited facilities often have lower complication rates and better patient outcomes.

Learn About Patient Experiences

Online reviews and surgeon ratings can provide insights into patients' experiences, but they often don’t tell the whole story. Watch patient videos, read testimonials, and hear directly from a patient about their experience with the surgeon and team. Here at NJBC, our monthly webinars give prospective patients the opportunity to hear directly from our current patients about their experience.

When you’ve done your research, understanding how to pick the right bariatric surgeon for you and asking the right questions can help you make an informed decision.

Will I Regain Weight After Stopping Prescription Weight Loss Medication like Wegovy® or Zepound™?

Estimated reading time: 3 minutes

FDA-approved prescription weight loss medications like Wegovy® and Zepbound™ can be effective tools on your weight loss journey. But what happens when you stop taking them? It’s a common question people have about regaining the weight they’ve lost. In this blog we'll discuss what to expect and a few tips to help you maintain your progress.

Why Weight Regain Can Happen

Weight loss medications are a tool for the treatment of obesity, not a cure. When the medications and healthy lifestyle changes are stopped, weight regain can occur. Think of obesity like other chronic conditions such as high blood pressure or diabetes - it requires ongoing management. Just as blood pressure medicine works best when combined with a healthy lifestyle, weight loss medications are similar. When paired with healthy eating habits, regular exercise and lifestyle changes, long term success is achievable. If you return to previous habits after stopping the medication, you're likely to regain weight.

How Prescription Medication Were Created to Work

Prescription medications like Wegovy® and Zepbound™ are designed to be used for long periods of time. They often take 5-6 months to reach full effectiveness. They work by altering the hunger pathways in our bodies, reducing cravings and slowing down the time it takes for our stomach to empty out. Usually when our body loses weight, it compensates by increasing hunger signals. These signals are blocked by weight loss medications.

When beginning a weight loss medication, we prescribe a starter dose. This helps your body get used to the medication and minimize potential side effects. As the medications begin to take effect, your provider will slowly increase the dosage. Typically patients reach the maximum dose around the 6 month mark. Once at the maximum dose, the medications are taken as a maintenance dose, which means that you continue at that dose until you no longer want to take the medication.

RELATED: Prescription Weight Loss Medication Options

The Importance of Lifestyle Changes

Habits matter. The medications work alongside lifestyle changes, not in place of them. Without a change in lifestyle habits such as mindful eating, portion control, and increased physical activity, when medications are stopped there is a higher chance for weight regain. At New Jersey Bariatric Center our medical weight loss program includes live nutrition and exercise classes that sets the foundation for  lifestyle changes.

Finding additional support from weight loss groups, family members and friends can also help you stay on track.

Readjusting Your Approach

Weight regain can feel discouraging but it does not mean that you or the medications have failed. It’s a temporary setback. Together, we can discuss options to restart your weight loss journey, from lifestyle changes and the MWL@NJBC program to weight loss surgery (if you qualify). We’re here to support you every step of your weight loss journey.

What is Food Noise and How Can I Silence It?

Estimated reading time: 3 minutes

Food noise is a term used to describe constant thoughts about food or a preoccupation with food that can be overwhelming or disruptive to daily life - that persistent inner voice chattering about cookies even though you just had lunch. When experiencing food noise you may not necessarily feel hungry, but you are still thinking or obsessing about food. It’s different from hunger. Hunger is physical whereas food noise is in the mind.

If you’re struggling with food noise, it’s important to know that you’re not alone. There are steps you can take to quiet the chatter and develop a healthier relationship with food.

Learn Your Physical Hunger Cues

Learn your body’s natural signals of hunger and fullness rather than relying on external cues. Pay special attention to your physical hunger cues so you can differentiate them from food noise cues. When you're physically hungry, practice mindful eating. I always recommend sitting down to eat at a table without watching TV or using a cellphone. Limit distractions to allow yourself to focus on the flavors of your meal and how you’re feeling while you’re eating. Savor each bite.

Healthy Food Habits

One way to regulate your hunger cues and food noise can be to adopt habits that help you make better food choices. If there’s one specific food that you simply cannot resist, try keeping it out of the house or out of your sight. Always have healthy snacks such as cut up vegetables, low-fat cheese sticks and yogurts on hand to snack on, and use smaller plates when possible, portions appear larger when on a smaller plate.

The  Importance of Sleep

Did you know not getting a good night’s rest can cause you to feel hungrier and consume more calories in a day? Leptin and Ghrelin are two hormones that help regulate hunger and satiety.  When you get a restful night's sleep, it allows your body to regulate the amount of these hormones.  Sleep can also impact how your body regulates insulin. Insulin is the hormone that helps regulate your blood sugar.  Making sure that you are active, getting outside and some sunlight daily can help you get good quality sleep as well. Aim for at leas 7 hours of sleep per night and try to go to bed and wake up at the same time each day to keep your body on a routine. Keep your bedroom dark and relaxing, put your phone down a couple of hours before bed, and avoid large meals too close to bedtime to regulate your sleep as well.

How Weight Loss Medication Can Help

Weight loss medications like Wegovy® and Zepbound™ can help quell the food noise. These medications are referred to as GLP-1 agonists. One of the ingredients in these medications, semaglutide, can affect the area of the brain that controls appetite. It works by targeting hormones in the brain and body that regulate hunger and satiety -the feeling of being full. It helps patients feel more in control, which enables them to make better choices without always feeling distracted by food. GLP-1 medications also make you feel full quickly, and with improved hunger control and food noise at bay, patients are able to make healthier choices with their meal.

Working with an RD

If you feel overwhelmed with food noise, I encourage you to make an appointment with a registered dietitian. Working with a registered dietitian can help you not only on your weight loss journey but also with anything that pops up along the way to your goals. We are able to tailor behavior modifications and offer solutions to individual dietary problems that you may have that will work for you.

Bariatric Surgery, Weight Loss Medications & You: Understanding Your Options

Both bariatric surgery - gastric sleeve and gastric bypass - and prescription weight loss medications are tools to help you reach a healthy weight; but they work differently and lead to varying amounts of weight loss. Here’s what you need to know about both options and how you can maximize your weight loss.

Understand How Each Helps You Lose Weight

Gastric sleeve and gastric bypass both create weight loss by decreasing the size of your stomach, which limits the amount of food you can eat at one time, leaving you feeling fuller sooner and longer.  Gastric sleeve jumpstarts weight loss by removing a portion of the stomach reducing the capacity for food intake, whereas gastric bypass works by both reducing the stomach size and rerouting the path food normally takes so that it “bypasses” a large section of the stomach and a portion of the intestine. Both decrease your levels of hunger hormones so you will feel less hungry. As you eat less food, your body will stop storing excess calories and start using its fat supply for energy.

Gastric sleeve and gastric bypass procedures result in significant long term, sustainable weight loss, on average between 60-80% of excess body weight. If you’re currently on weight loss medications and are looking for  long term, sustainable results change, bariatric surgery might be the right fit for you. To calculate your potential weight loss with gastric sleeve, click here. For gastric bypass, click here

Prescription weight loss medications aid in weight loss by decreasing hunger hormones while simultaneously increasing feelings of satiety, leading to reduced food intake ultimately leading to weight loss. Qsymia® (phentermine & topiramate), Saxenda® (liraglutide), Wegovy® (semaglutide) and Zepbound™(tirzepatide) are FDA-approved weight loss medications prescribed in either weekly/daily injections or capsules. Medications can typically lead to a total body weight loss ranging between 10-20%. If you’ve already had bariatric surgery and gained some of the weight back, medical weight loss might be a good option for you, if you qualify.. To calculate your potential weight loss with prescription medications, click here

Do I Qualify?

For weight loss surgery, patients qualify with a body mass index (BMI) of 40 and above, or 35 and above with obesity-related conditions such as high blood pressure, diabetes, or obstructive sleep apnea. Find out if you qualify and calculate your BMI here.

For prescription weight loss medication patients qualify with a (BMI) of 30 and above, or a BMI of 27 and above with obesity-related conditions mentioned above. Find out if you qualify for medical weight loss here.

Costs and Insurance Coverage

For many people, weight loss surgery is an affordable health option because it is covered by their health insurance plan. However, while medical benefits usually cover the majority of the surgical costs, there may be out-of-pocket expenses depending on the procedure and your specific policy. Since coverage for each individual policy varies by company and employer, our staff will determine your exact coverage prior to your first appointment. Our navigators have a very high authorization success rate and regularly work with many insurance companies.

While some insurance plans cover weight loss surgery, coverage for prescription weight loss medications may vary. It’s important to contact your insurance provider beforehand to find out if you have coverage for weight loss medications. If you do not have coverage, self-pay options are available.

Your Weight Loss Journey At NJBC

The journey to weight loss doesn’t end when you decide on weight loss surgery or medication. Both medical weight loss and surgical patients start the process with routine lab work for our physicians to gain a better understanding of their health. This information along with your weight loss goals helps our doctors develop a plan that’s right for you.

With weight loss surgery you will meet with your surgeon, and then be teamed up with an experienced patient navigator who will be your guide, scheduling preoperative appointments, answering questions and ensuring all insurance and clinical requirements are met. After surgery, at your follow-up appointments our team will help you adjust to your new lifestyle.

With prescription weight loss medications, once you have verified your medication coverage with your insurance provider and completed routine lab work you’ll meet with our doctor to review lab test results and medication options. Once a medication is prescribed, NJBC’s medical team - registered dietitians and physician assistants - will meet with you monthly to monitor and review your progress.

We will provide guidance on nutrition and exercise, support you throughout the entire journey, including plateaus and possible weight regain and we’ll be there to celebrate all of your achievements.

Getting Started on Your Weight Loss Journey At NJBC

Both medical weight loss and surgical patients start the process with routine lab work for our physicians to gain a better understanding of their health. This information along with your weight loss goals helps our doctors develop a plan that’s right for you.

With weight loss surgery you will meet with your surgeon, and then be teamed up with an experienced patient navigator who will be your guide, scheduling preoperative appointments, answering questions and ensuring all insurance and clinical requirements are met. After surgery, at your follow-up appointment our team will help you adjust to your new lifestyle.

With prescription weight loss medications, once you have verified your medication coverage with your insurance provider and completed routine lab work you’ll meet with our doctor to review lab test results and medication options. Once a medication is prescribed, NJBC’s medical team will meet with you monthly to monitor and review your progress.

Maintaining Your Weight Loss Progress Through Thanksgiving

Thanksgiving is all about gathering together, giving thanks and enjoying a meal with family and friends. However, this big meal can be quite challenging and stressful if you’re trying to lose weight or maintain your weight loss progress. Heading into Thanksgiving with a solid plan is a great way to help you stay on track. Here are 4 tips that we tell our patients.

Plan Ahead

Create a meal plan that includes healthy options for breakfast, lunch, dinner and dessert. This will help you make mindful choices when you’re faced with temptation especially at Thanksgiving dinner. Our registered dietitian team is always here to support you in creating a customized plan that works for you.

Avoid Skipping Meals

On Thanksgiving day avoid skipping meals. Treat the day just like any other day of the week. Opt for a healthy protein packed breakfast such as a smoothie or an omelet. For lunch, focus on lean protein and veggies like chicken breast and a salad. Overall, remember to stay fueled throughout the day instead of starving yourself until the big meal. Most importantly, listen to your body. Once you’re full, practice pushing your plate away or packing up leftovers. You should leave the table feeling comfortable and satisfied -- not stuffed.

Build Your Plate Mindfully

Protein should be the foundation of your meals while on a weight loss journey. It helps to maintain lean body mass and to keep you feeling fuller, longer.. I recommend filling your holiday plate with protein first and allocate 50% percent of your plate to it for an adequate amount of nutrients. Next addition to your plate should be veggies which provide your body with antioxidants and vitamins A, C and E.  Some good options include sautéed carrots and green beans, a spinach salad or roasted broccoli. For higher carb options such as stuffing or mashed potatoes opt for a smaller spoonful and enjoy in moderation. Remember that it’s okay to indulge a bit but just be mindful of your portion sizes, eat slowly and mindfully.

Add some Movement to Your Day

Getting up from the table and away from the food is a good way to ensure that you won’t be tempted to overindulge. We recommend going for a walk after dinner or rallying up some company for a sporting activity. Moving will not only boost digestion but also helps to burn some calories.
